Purpose:
To be Fully Informed before consenting or not consenting to the COVID vaccine.
Your voluntary, informed consent is needed before any medical procedure under the Australian Charter of Healthcare Rights. The Dept of Health has provided you with a visit to your GP to answer your questions about the health risks of the covid vaccine. The Prime Minister also encourages you to do this.
These documents will help you prove your attempt at giving informed consent.
Print out the questions, take them with you to your consultation. Write down their answers and if they can’t answer them to your satisfaction, date and sign it and ask them to put it on your file as proof that you had your ‘informed consent’ consultation. Provide a copy to your employee or whoever is trying to coerce you.

3. Bio-distribution – Pfizer (document to leave with your GP for his own consideration)
4. Mandatory Jab Template – Legal letter that can be edited and used to give to your employer.
